From Hypertension to Heart Failure

Abstract
Hypertension is the most common risk factor for hypertensive heart disease (HHD), including left ventricular hypertrophy
(LVH) and heart failure. LVH is a maladaptive process that is composed of an increased myocardial oxygen demand, impaired epicardial coronary perfusion and ventricular fibrosis and remodeling, and the final result is systolic and diastolic dysfunction. Echocardiography can play a key role for making the early diagnosis of HHD. Early and aggressive treatment of hypertension prevents the development of new-onset heart failure and it decreases the cardiovascular morbidity and mortality. Lifestyle modification and pharmacotherapy are the cornerstones for achieving optimal blood pressure control and to prevent progression to HHD. The treatment of systolic heart failure is well established, yet the efficacy of the treatment for heart failure patients with a preserved LV ejection fraction has not been clearly established. Thus, it is necessary to conduct future studies on this field.
